All new student registrations for the current or upcoming school year must be completed online.

Required Documents Needed to Complete Registration
Current Mortgage/Settlement Statement OR Lease Agreement
Current Electric Bill
Current Other Utility Bill (water, gas or cable)
Driver’s License (verification purposes)
Student’s Birth Certificate (original or certified copy)
Student’s Physical Health Form (dated within 12 months of enrollment)
“Immunization Record” MUST BE on the Florida 680 Immunization Form
Once you have completed the online registration process, you need to contact the school to complete the registration process. Please contact Carrie Ortbals, Guidance Secretary at [email protected] or (904) 547-8406 to schedule an appointment to sit down with a guidance counselor and select classes. Please bring your child’s final report card showing promotion from their previous school and any Gifted, ESE, IEP or 504 documents.
